Who We Are
Excelsior Wellness is a nonprofit healthcare system comprised of integrated behavioral health, primary care, and young adult services who provide comprehensive wraparound services to meet the needs of our community.
In every aspect of care, we seek to create partnerships for wellness and recovery. We put patients at the center of care and aim to be trusted partners on their wellness journey.

- Excelsior Family Medicine – Person-Centered Primary Care and Foster Care Center of Excellence
- Excelsior Wellness Center – Community Behavioral Health Including Wraparound Care for Children, Young Adults, and Their Families
- Excelsior Integrated Care Center – Higher Level of Care Including Bridge Housing for Young Adults
